Ignite-UX on 11iv3 Itanium host - make_tape_recovery

I run make_tape_recovery process on all my hosts against the root volume group. On one of my hosts the process ran fully but at the end reported this..
THE FOLLOWING LINE(S) HAD TOO FEW/INCORECT ENTRIES:
LINE 5 tmp/ 40770 1299092416 96 0 NA 3 0x52b 0x40000004

Then it says archive created succcessully.
Can anyone shed some insight on the the message above. Is 0x40 the minor number?

This typically occurs because the filename has spaces or other junk in it. The "5" above is the length of the filename and I'm not sure how that "/" got there.

Thanks for the helpful info I was able to locate ths issue (filename). !!!
