make_tape_recovery spits out i/o error after "Wrtiing boot LIF to tape" ???

make_tape_recovery -a /dev/rmt/1mn -I -v -x inc_entire=vg00  -> command use to backup.

I have tape in the drive. wondering what is causing the i/o error.

I/O error

Check your tape. Clean the tape drive.  Try a new tape.

It did not successfully write the boot lif to tape.  Notice the "I/O error".
