PDrv x:y at startup is in FAILED state
i have removed with irm a phisical disk from my system, and everything seems correct in irm and in the system, but at the end of startup the system gives four error messages related to disk that i have removed:May 25 17:31:18 chptest syslog: IRMD[Severe]: Adapter 0/4/2/1 PDrv 2:9 at startup is in FAILED State
May 25 17:31:19 chptest syslog: IRMD[Severe]: Adapter 0/4/2/1 PDrv 2:11 at startup is in FAILED State
May 25 17:31:20 chptest syslog: IRMD[Severe]: Adapter 0/4/2/1 PDrv 2:13 at startup is in FAILED State
May 25 17:31:21 chptest syslog: IRMD[Severe]: Adapter 0/4/2/1 PDrv 2:15 at startup is in FAILED State
Is there anything that i may have forgotten to do to clean everything?
Do you have any suggest?

did you remove the associated LUN?

We have an inconveniant with IRMD when we diagnosing two (2) rp5470 L3000 HP 9000 servers(HPUX 11i V1):
attached to DS2300 (4 disks 36GB online) ==>DB active and running well!
this message appears on the first server when we tested vai irm tool and rebooted the system
we have done an IGNITE successufly too!
we get mail message system on the first server:
IRMD[Severe]: Adapter 0/12/0/1 PDrv 0:0 at startup is in FAILED State...
the first server appears with ioscan as bellow:
# ioscan -fnC disk
