/dev/dsk/disk_query

I have a new disk put into a rp3440 running 11.23. Before replacement I vgreduced the disk out of vg00. When I rebooted, server crashed, had to boot with IPL> hpus -lq.
Now, disk has device file of /dev/dsk/disk_query.

Any ideas on how to clear this and get disk back into vg00? No mirroring, as this is a play box.

As said in your previous thread - check for SAM patches and / or hung SAM processes.

No hung SAM processes, and have patch PHCO_34208 cumulative SAM patch installed.

so, how does one clear the /dev/dsk/disk_query flag if all looks ok within SAM?

Now I'm getting this error when trying to mount:

4pa(root):/home/root#mountall

full file system check required, exiting ...

Any ideas?

This patch has several newer versions:

However, consider to delete this file.

I don't think this is a SAM issue, as I have managed to clear the disk_query file. I have added the new disk into vg00, and so on. Just need to get this FS sorted out.
