Hi,
I check the /var/adm/syslog/syslog.log and find the following erorr:

Jul 17 16:46:32 server1 vmunix: SCSI: Read error -- dev: b 31 0x01e000, errno: 126, resid: 2048,
Jul 17 16:46:37 server1 above message repeats 94 times
Jul 17 16:46:32 server1 vmunix: blkno: 8, sectno: 16, offset: 8192, bcount: 2048.
---------------clip---------------------
Can you give me any suggestion? Does any HP documents explain these error codes?

the device is c1t14d0 which gives this read error on server1. check this out.

You need to check the event (with the timestamp Jul 17 16:46:32) on /var/opt/resmon/log/event.log
From the log, the device should be c1t14d0. You can check it with the command below :
# ll /dev/dsk | grep 0x01e000
Check the disk with commands below :
# ioscan -nfCdisk
# diskinfo -v /dev/rdsk/c1t14d0
If the disk with NO_HW from ioscan command and there's no info comes from diskinfo, the disk is faulty already and need to replace.
