Did you put the cables back in in the same cards they came from?
Basically, you have no disk to boot with.
The trigger appears to be the cable removal and plug in. I'm just wondering if anything else like patching was done on this system. A reinstall of PHKL_18543 on an 11.00 system will get these exact results, because you can't reinstall that patch.
A rezoning of the fiber switch of failure of the fiber switch, or changing world wide name ownership on the LUN/disks on the disk array could cause this.

Ensure that the LUN addressing is right and unique. Check with your storage people.

Boot the m/c normally with one cable, install the recent patch kit (from support plus media) and connect the second cable.
