sorry for the late response..

diskinfo output is
# diskinfo /dev/rdsk/c1t4d0

SCSI describe of /dev/rdsk/c1t4d0:
vendor: SEAGATE
product id: ST39173WC
type: direct access
size: 0 Kbytes
bytes per sector: 0

the size is 0 bytes

The disk has gone bad.

check the size,it shows '0' bytes.

Replace the disk.

Yep, that disk is history.
Call the RC & log a HW case.
