HPE 1/8 G2 autoloader

Hello, please assist with the issue below.
For last two weeks we face problem while transferring backups to HPE 1/8 G2 tapes. There are two main problems:
1) FAILED: After starting job to write backups to tape we get failed status in Veeam Backup & Replication and cartridge of tape switches to protected or offline mode. When this happens firstly we unprotect that cartridge and reboot tape device, sometimes 4-5 reboots needed to finish backup successfully.

Logs from tape:

21.09.19 04:56:07.26  Libctrl task Error - HE: elevator blocked

     Return status: 0x80 - command failed

     Global error code: 0x8B - HE: elevator blocked

     Robotic detailed info: 0x55 - error during elevator movement in drive position

     Current command: 0x15 - move cartridge

     Task originator: 0x02 - CDB interpreter task

     Extender library specifier: 0x00, 0

     Source element type.: 0x02 - storage slot

     Element number: 0x06, 6

     Destination element type.: 0x04 - tape drive

     Element number: 0x01, 1

     Component ID type: 0x02 - medium PVT (barcode)

     Component ID: DUS323L6

2) WARNING:  Sometimes writing to tape finishes successfully but with warning тАЬThe library was unable to read the bar code on a cartridgeтАЭ.

What is the system model?

I think the barcode reader went faulty. Check for any visible damages, dust, scratches or wear&tear on barcode reader.
