      <description>one of our disks on path:&lt;BR /&gt;&lt;BR /&gt;0/5/0/0.8.0 is giving "information failed" on the stm tool.&lt;BR /&gt;&lt;BR /&gt;however i don't get "verified failed" or "excercise fail" and neither do i get any errors with ioscan -fnC disk?&lt;BR /&gt;&lt;BR /&gt;what could be going on?&lt;BR /&gt;&lt;BR /&gt;thanks,&lt;BR /&gt;&lt;BR /&gt;ap.</description>

      <description>actually i did some more checking up:&lt;BR /&gt;&lt;BR /&gt;last week 1/12/0/0.8.0 failed and an hp h/w support did a replacement.&lt;BR /&gt;&lt;BR /&gt;1/12/0/0.8.0 under vg02 had lvol3, lvol5 and lvol6&lt;BR /&gt;&lt;BR /&gt;i noticed that 1/5/0/0.8.0 is the mirror for the 1/12/0/0.8.0&lt;BR /&gt;&lt;BR /&gt;could there be a synch issue?&lt;BR /&gt;&lt;BR /&gt;thanks,&lt;BR /&gt;&lt;BR /&gt;ap.</description>

      <description>Yes, you definitely need to upgrade to a recent version; the last version for 11.00 was A.44.00.  You should then install PHSS_32921 on top of that.&lt;BR /&gt;&lt;BR /&gt;You can get A.44.00 from &lt;A href="http://h20293.www2.hp.com/portal/swdepot/displayProductInfo.do?productNumber=B6191AAE" target="_blank"&gt;http://h20293.www2.hp.com/portal/swdepot/displayProductInfo.do?productNumber=B6191AAE&lt;/A&gt;&lt;BR /&gt;&lt;BR /&gt;If you still see the problem, look in the Activity and Failure logs for the disk info tool in STM to see if there are any clues there as to why the tool failed, but the upgrade is the first thing to do.&lt;BR /&gt;&lt;BR /&gt;Andrew</description>
