      <description>All seems fine except disk-"/dev/dsk/c0t0d0"&lt;BR /&gt;Is there a disk like that and present?&lt;BR /&gt;&lt;BR /&gt;Also, you should not be running vgscan -a directly. run vgscan -pva first, then if required run vgscan -a. Also backup old file first.</description>

      <description>Did you loose old lvmtab? Is vg01 and vg02 active when you are running vgscan?&lt;BR /&gt;&lt;BR /&gt;A good sysadmin always backs up a file before doing anything to it.</description>

      <description>If you do vgscan -pva, does it show details for vg01 and vg02? What does dmesg and syslog.log say?&lt;BR /&gt;&lt;BR /&gt;What hardware errors/defects you noticed?</description>
