    <description>Rajeev,&lt;BR /&gt;&lt;BR /&gt;the line May 5 05:54:32 germany vmunix: bp-&amp;gt;b_dev: 1f04c000&lt;BR /&gt;&lt;BR /&gt;from your syslog output will point you to the problem device. its a disk.&lt;BR /&gt;&lt;BR /&gt;ll /dev/dsk | grep 1f04c000&lt;BR /&gt;&lt;BR /&gt;May 5 05:54:32 germany vmunix: lsp-&amp;gt;state: 5&lt;BR /&gt;most of the time state 5 refers to device offline.&lt;BR /&gt;&lt;BR /&gt;please chck the diskinfo, dd etc to check if the disk is OK.&lt;BR /&gt;&lt;BR /&gt;Good Luck.&lt;BR /&gt;&lt;BR /&gt;Ajit</description>

      <description>Hi,&lt;BR /&gt;&lt;BR /&gt;Try &lt;BR /&gt;&lt;BR /&gt;ioscan -fCdisk ( if you find a disk UNCLIMBED) better raise a call with HP to get it replaced&lt;BR /&gt;&lt;BR /&gt;I guess one of the SCSI disk is down or a cable not connected properly.&lt;BR /&gt;&lt;BR /&gt;Chan</description>

      <description>Hi Rajeev,&lt;BR /&gt;&lt;BR /&gt;That device maps to c4t12d0&lt;BR /&gt;But it could be a transient condition.&lt;BR /&gt;Make sure it doesn't resolve as UNCLAIMED on an ioscan.&lt;BR /&gt;&lt;BR /&gt;Rgds,&lt;BR /&gt;Jeff</description>
