I had faced an problem of hardware path failure 4_0_1_1_0_4_0 Investigate and found that :

Also all fibre card are in Online State.

I checked with storage team, they are telling that all things are fine. It appears that some removed LUNs have not been deleted from host level volume management tool. Please check & confirm.

can you suggest how can i proceed

Are you using vxvm or lvm? Could you post the command output from the volume manager you are using?

vxvm
# vxdisk list

lvm
# vgdisplay -v

As noted.

These lun presentations look to be removed. This removal is done either in the switch zoning, array configuration, or both.

Does not matter what you have or do at the OS side.

Review the array and SAN configurations.

Hi,

Check for all your FS, whether they all are ok. If all are ok.

Then contact for storege team whether they have assigned any luns to this server and taken back.

This clearly shows that LUNS of XP storage with OPEN-V emulation are decommisioned from storage side. Please ask them to check again.

I checked and found that the Fibre card attached with the path is ONLINE

Topology = PTTOPT_FABRIC
Driver state = ONLINE
Hardware Path is = 4/0/1/1/0/4/0

LUNS are removed from command view. Like logical removal. FC is online because prysical cable connections are intact.

check mac address of UR FC . go on command view make sure that host group and mac address are OK.

Hi Indrajeet,

You can check the WWN (or mac address) of fibre card using below command.

# /opt/fcms/bin/fcmsutil

It seems some LUNs from Hitachi Storage which were previously allocated to this server have been deallocated from the storage side. Check with your storage team, if the LUNs have been deallocated because they were no longer been used, then you can plan for a reboot of the server which will clean up your ioscan output.

I think the vital question is, do you still need those luns, or can you remove these devices/luns from vxvm and the OS?
