Probably create a symlink from /opt/XX to /var/opt/XX and install as normal.

>I can create a folder perl5 in /opt and link that folder in /var/opt.

Well, you create a symlink in /opt/, not a directory:
ln -s /var/opt/perl5 /opt

>when I install the perl, will it goes in that pre-existing folder?

Yes, it should. swinstall doesn't remove the directory before installing a new one.
