    <description>Hi guys,&lt;BR /&gt;In my hp-ux 9000 712/100 Workstation, I sometimes get the following error when I switch it on:&lt;BR /&gt;System console is on the ITE&lt;BR /&gt;logical volume 64, 0x3 configured as ROOT &lt;BR /&gt;error writing VGDA [1] to disk H/W path 2/0/1.6.0 (error = 5)&lt;BR /&gt;LVM: Lost quorum in vg[0], too many PVs missing! logical volume 64, 0x2 configured as SWAP&lt;BR /&gt;&lt;BR /&gt;However, I have to crash the system and start it again and it boots without the error msg.&lt;BR /&gt;please advise.&lt;BR /&gt;Thanks in advance.</description>

      <description>Hi XMAN,&lt;BR /&gt;&lt;BR /&gt;You're root/boot mirror disk at 2/0/1.6 is failing.&lt;BR /&gt;You should replace it before the other fails &amp;amp; you're out of the game.&lt;BR /&gt;&lt;BR /&gt;Note: When you rebuild the boot mirrors after replacement, use:&lt;BR /&gt;mkboot -a "hpux -lq" /dev/rdsk/cxtydz&lt;BR /&gt;on the root/boot mnirrors to facilitate booting when a mirror member is lost.&lt;BR /&gt;&lt;BR /&gt;Rgds,&lt;BR /&gt;Jeff</description>

      <description>Yea...hope you got a good price, because the bad news is if you want to retain a root/boot mirror, you'll need to obtain the disk from HP as boot disks need the HP firmware to boot properly.       <description>Thanks jeff again. &lt;BR /&gt;What I understood that I can not just buy the HD, install it and load it with the ignite tape that I've created!! it that what you mean, is there any other recovery procedures that I'm not aware of?&lt;BR /&gt;</description>
