Nope, I disk shutdown the disk array.I only restarted the server yesterday due to a high disk IO.

What is the current status of the device in this path 0/0/6/0/0/0/0/4/0/0/0?

ioscan -fnH 0/0/6/0/0/0/0/4/0/0/0

What is the device id of this FC (if it is) ?

i.e. /dev/fcdx ?

The missing device is always

0x500110a00089ceb3
and
0x500110a00089ceb2

this means one EVA 4400 controller.

IMHO at least this controller was down.

Ask your storage folks.
