    <description>&lt;P&gt;Hello,&lt;/P&gt;&lt;P&gt;&amp;nbsp;&lt;/P&gt;&lt;P&gt;I'm not sure what happened with this server as I was not the first port of call to fix it as it's in our US data center, but we managed to get it working using the below syntax:-&lt;/P&gt;&lt;P&gt;&amp;nbsp;&lt;/P&gt;&lt;P&gt;# vgcfgrestore -n vg00 /dev/rdsk/c2t0d0&lt;/P&gt;&lt;P&gt;&amp;nbsp;&amp;nbsp;&amp;nbsp;&amp;nbsp;&amp;nbsp; This will restore the LVM headers to the new disk.&lt;/P&gt;&lt;P&gt;&amp;nbsp;&lt;/P&gt;&lt;P&gt;Next proceed to re-activate the VG:&lt;/P&gt;&lt;P&gt;&amp;nbsp; # vgchange -a y vg00&lt;/P&gt;&lt;P&gt;&amp;nbsp;&amp;nbsp;&amp;nbsp;&amp;nbsp;&amp;nbsp; You should now see the disk as part of the VG again.&lt;/P&gt;&lt;P&gt;&amp;nbsp;&lt;/P&gt;&lt;P&gt;Assuming this is a mirrored boot disk then proceed with the mkboot commands:&lt;/P&gt;&lt;P&gt;&amp;nbsp;&amp;nbsp;&amp;nbsp;&amp;nbsp;&amp;nbsp; # mkboot -l /dev/rdsk/c2t0d0&lt;/P&gt;&lt;P&gt;&amp;nbsp;&amp;nbsp;&amp;nbsp;&amp;nbsp;&amp;nbsp; # mkboot -a "hpux -lq" /dev/rdsk/c2t0d0&lt;/P&gt;&lt;P&gt;&amp;nbsp;&lt;/P&gt;&lt;P&gt;Reset the root/boot/dump/swap assignments:&lt;/P&gt;&lt;P&gt;&amp;nbsp;&amp;nbsp;&amp;nbsp;&amp;nbsp;&amp;nbsp; # lvlnboot -R&lt;/P&gt;&lt;P&gt;&amp;nbsp;&amp;nbsp;&amp;nbsp;&amp;nbsp;&amp;nbsp; # lvlnboot -v&amp;nbsp;&amp;nbsp;&amp;nbsp;&amp;nbsp; (view to confirm)&lt;/P&gt;&lt;P&gt;&amp;nbsp;&lt;/P&gt;&lt;P&gt;Start the sync of any mirrored logical volumes:&lt;/P&gt;&lt;P&gt;&amp;nbsp;&amp;nbsp;&amp;nbsp;&amp;nbsp;&amp;nbsp; # vgsync /dev/vg00&lt;/P&gt;&lt;P&gt;&amp;nbsp;&lt;/P&gt;&lt;P&gt;Thanks to all who responded :-)&lt;/P&gt;&lt;P&gt;&amp;nbsp;&lt;/P&gt;&lt;P&gt;cheers&lt;/P&gt;&lt;P&gt;&amp;nbsp;&lt;/P&gt;&lt;P&gt;nick&lt;/P&gt;</description>

      <description>&lt;P&gt;Hello,&lt;/P&gt;&lt;P&gt;&amp;nbsp;&lt;/P&gt;&lt;P&gt;I have a server which has has it's mirror disk fail, the disk has now been replaced but I'm still having issues getting the mirroring set back up.       <description>&lt;P&gt;If you can find the "When Good Disks Go Bad" whitepaper, that is an immense help when replacing a disk.&lt;/P&gt;&lt;P&gt;&amp;nbsp;&lt;/P&gt;&lt;P&gt;In your situation, the steps I would do are:&lt;/P&gt;&lt;P&gt;&amp;nbsp;&lt;/P&gt;&lt;P&gt;--Verify the disk is CLAIMED in 'ioscan -fnC disk'&lt;/P&gt;&lt;P&gt;&amp;nbsp;&lt;/P&gt;&lt;P&gt;# vgcfgrestore -n vg00 /dev/rdsk/c2t0d0&lt;/P&gt;&lt;P&gt;# vgchange -a y vg00&lt;/P&gt;&lt;P&gt;# vgdisplay -v vg00&lt;/P&gt;&lt;P&gt;(check and make sure /dev/dsk/c2t0d0 shows as 'available' in the output)&lt;/P&gt;&lt;P&gt;# mkboot /dev/rdsk/c2t0d0&lt;/P&gt;&lt;P&gt;# mkboot -a "hpux -lq" /dev/rdsk/c2t0d0&lt;/P&gt;&lt;P&gt;# vgsync vg00&lt;/P&gt;&lt;P&gt;&amp;nbsp;&lt;/P&gt;&lt;P&gt;The vgsync will resync the mirrors between your 2 disks.&lt;/P&gt;</description>

      <description>You maybe rebooted with the bad disk, so this disk is not listed in "vgdisplay" at all. Likely you performed some steps to remove the disk already (usually not needed at all).&lt;BR /&gt;&lt;BR /&gt;Perform the suggested steps and do&lt;BR /&gt;&lt;BR /&gt;# vgdisplay -v vg00&lt;BR /&gt;&lt;BR /&gt;# lvlnboot -v&lt;BR /&gt;&lt;BR /&gt;and post.&lt;BR /&gt;&lt;BR /&gt;&lt;BR /&gt;&lt;BR /&gt;If you have problems to do a "vgcfgrestore", you need to do a "pvchange -a n ..." first.</description>
