The disk is alive, and the driver works, that is the conclusion form being present in the ioscan -fnCdisk list.
But the "internal format" is what it was in the HP Model-20 (nike-clarion) raid stripe configuration.
And next: it is not a supported HP disk
SAM fails at the pvcreate, manual it shows:
# pvcreate /dev/rdsk/c4t4d0
pvcreate: Couldn't open physical volume "/dev/rdsk/c4t4d0":
No such device or address
#
Any suggestions?

You may wish to try the 'mediainit' command on the disk. I don't have the syntax for your right now, but there is a man page for it.

One other thing I've seen before (not for a while) and that is sometimes a disk will will think it is write-protected for no apparent reason. Try the mediainit command first....

Cheers
~Michael~

Wile trying the /etc/disktab option, I also tried the mediainit. Could not open the device file either.
THE FINAL SOLUTION WAS:
Formatted the drive on a W98PC (in dos mode) using SCSIFMT.EXE (low level formatter) from the EZ-SCSI LITE V4.0 floppy's which were part of the adaptec 1510 scsi controller kit.
Lukky I still had this old (ISA) stuff.
After that I could add the SE drives without any problem using SAM to this 11.11 G40.
Thanks for all who cooperated on this!
