      <description>Hi,&lt;BR /&gt;&lt;BR /&gt;It looks one of the physical volume is missing on which the primary copy of lv is residing.&lt;BR /&gt;&lt;BR /&gt;&lt;BR /&gt;you need to find the pv key to reduce the stale copy.&lt;BR /&gt;&lt;BR /&gt;#lvdisplay -v -k /dev/vg_rac/ora_ocr&lt;BR /&gt;&lt;BR /&gt;(note down the PV KEY under second column, which was showing PX_NOPV)&lt;BR /&gt;then remove the primary copy of the lv using pvkey.&lt;BR /&gt;&lt;BR /&gt;#lvreduce -m 0 -k /dev/vg_rac/ora_ocr &lt;PVKEY&gt;&lt;BR /&gt;&lt;BR /&gt;Now lv should have one copy of data without stale. check with lvdisplay.&lt;BR /&gt;&lt;BR /&gt;#lvdisplay -v /dev/vg_rac/ora_ocr&lt;BR /&gt;&lt;BR /&gt;Now you need to fix the disk issue which is missing and remirror it.&lt;BR /&gt;&lt;BR /&gt;Hope it helps&lt;/PVKEY&gt;</description>

      <description>of course, c1t2d0s3 could have failed as well.&lt;BR /&gt;&lt;BR /&gt;check the results of ioscan to see if you've any NO_HDW indications</description>
