    <description>The failed disk information is not present in kernel hence you are getting these messages. I would suggest you to confirm the following things:&lt;BR /&gt;&lt;BR /&gt;check vgdisplay o/p for the following:&lt;BR /&gt;&lt;BR /&gt;Cur PV                      2&lt;BR /&gt;Act PV                      2&lt;BR /&gt;&lt;BR /&gt;I am quite sure that both the above values will not match. Does it show the dailed disk in vgdisplay -v o/p ??&lt;BR /&gt;&lt;BR /&gt;Have you replaced the failed disk and after that you started getting above error messages?? If so then try restoring the vg configuration on the replaced disk using vgcfgrestore -n vg00 &lt;DISK device="" name=""&gt;&lt;BR /&gt;&lt;BR /&gt;After this execute vgchange -a y vg00.&lt;BR /&gt;&lt;BR /&gt;I hope you are following all the steps for mirroring the root disk.       <title>Mirror disk replacement</title>

      <description>If you have not replaced the disk then how did you find that this disk has failed ??&lt;BR /&gt;&lt;BR /&gt;There is a possibility that only LVM header information is corrupt and that may get resolved after doing vgcfgrestore and vgchange.&lt;BR /&gt;&lt;BR /&gt;If that doesn't resolves the problem then do the following:&lt;BR /&gt;&lt;BR /&gt;1. Rename the lvmtab file.&lt;BR /&gt;mv /etc/lvmtab /etc/lvmtab.20080704&lt;BR /&gt;&lt;BR /&gt;2. run vgscan&lt;BR /&gt;vgscan -av&lt;BR /&gt;&lt;BR /&gt;3. Take vgcfgbackup&lt;BR /&gt;&lt;BR /&gt;This will populate the lvmtab with current information and there won't be any error message.&lt;BR /&gt;&lt;BR /&gt;Then later on you can proceed with adding mirror disk as a fresh task.&lt;BR /&gt;</description>

      <description>In addition to above, I found that following fcp domains are NO_HW. You should check the connectivity of fibre cable to FC Switch/ Storage. However it is not related with this disk trouble since both the internal disk are CLAIMED.</description>
