system is running hpux 11.11 and PHKL_33356 is already installed. system is connectd to a SAN.

any ideas?

These errors are not vxvm related but are reported by the sdisk device driver for a bad disk. The major and minor number "b 31 0x060100" translates to the block special file "/dev/dsk/c6t0d1". Check with dd if "/dev/dsk/c6t0d0" is okay.
