    <description>thanks for ideas,&lt;BR /&gt;problem found (of course,somewhere else)&lt;BR /&gt;&lt;BR /&gt;RAID 7 has been connected to the system in wrong way (terminator and scsi cable)&lt;BR /&gt;&lt;BR /&gt;Thanks for ideas anyway&lt;BR /&gt;regards&lt;BR /&gt;&lt;BR /&gt;Michal</description>

      <description>Hi folks,&lt;BR /&gt;after I boot my server, following message has appeared and system halted&lt;BR /&gt;&lt;BR /&gt;swap device table (start&amp;amp;size given in&lt;BR /&gt;512 byte blocks)&lt;BR /&gt;entry 0 major nmber is 64,minor is 0x2;LVM:vg[0]:pvnum=0 (dev_t=0x1c015000) is POWERFAILED&lt;BR /&gt;&lt;BR /&gt;what is wrong ??&lt;BR /&gt;rgards&lt;BR /&gt;thanks&lt;BR /&gt;&lt;BR /&gt;Michal</description>

      <description>the disk at teh address supplied in vg0 (root vg) is turned off or has failed or is unreachable.&lt;BR /&gt;Check your disc at that hardware address related to the dev_t entry</description>

      <description>I agree with Melvin  ... H/W is the most likely cause.&lt;BR /&gt;&lt;BR /&gt;However we don't no much about your setup ... &lt;BR /&gt;&lt;BR /&gt;is this a MC/SG cluster or just one server ?&lt;BR /&gt;&lt;BR /&gt;any additional clues in syslog.log ? &lt;BR /&gt;&lt;BR /&gt;Do any other luns/devices report POWERFAILS at any othertime?&lt;BR /&gt;&lt;BR /&gt;What is the pvtimeout set to for each of the PV's? &lt;BR /&gt;The default is 30secs which in the case of arrays and a heavy I/O load, could be too short.&lt;BR /&gt;Values such as 60,90 or even 120secs are not uncommon.... may be worth a try before going down the h/w route ?</description>
