      <description>Guys,&lt;BR /&gt;&lt;BR /&gt;When I perform the following command:&lt;BR /&gt;&lt;BR /&gt;dd if=/dev/rdsk/c0t6d0 of=/dev/null bs=1024&lt;BR /&gt;&lt;BR /&gt;I get the following output:&lt;BR /&gt;&lt;BR /&gt;dd read error: I/O error&lt;BR /&gt;6930+0 records in&lt;BR /&gt;6930+0 records out&lt;BR /&gt;&lt;BR /&gt;What could be the problem and how would I resolve this.&lt;BR /&gt;&lt;BR /&gt;Thanks in advance</description>

      <description>Hi,&lt;BR /&gt;&lt;BR /&gt;Check the disk is claimed in an ioscan.&lt;BR /&gt;Check syslog.log for any error messages.&lt;BR /&gt;&lt;BR /&gt;It does look as though you have a faulty disk.&lt;BR /&gt;&lt;BR /&gt;Regards,&lt;BR /&gt;&lt;BR /&gt;Hilary</description>

      <description>Guys&lt;BR /&gt;&lt;BR /&gt;Please find attached the ioscan (no real problems in that)  i ahve also ran cstm on the devices, doesn't show any problems!&lt;BR /&gt;&lt;BR /&gt;All the lvols on that disk are fine.....???  and u guys are right may need disk replacing!&lt;BR /&gt;&lt;BR /&gt;let meknow if there are ant other tests that can be carried out!</description>

      <description>&lt;BR /&gt;Try using mediainit on your disk to low level format it in an attempt to fix the bad block;&lt;BR /&gt;&lt;BR /&gt;mediainit -v /dev/rdsk/c0t6d0&lt;BR /&gt;&lt;BR /&gt;This is a DESTRUCTIVE disk so the disk cant be in use at the time - remove it from any volume group. If it succeeds then your disk is aok - pvcreate -f it and add it back into a VG and reuse. If mediainit fails your disk is dead and must be replaced.&lt;BR /&gt;</description>

      <description>Is the disk available in LVM ?&lt;BR /&gt;&lt;BR /&gt;We have had problems in the past, where the disk is calimed in an ioscan, but unavailable in LVM.&lt;BR /&gt;In this instance we replace the disk &amp;amp; re-sync the mirrors.&lt;BR /&gt;&lt;BR /&gt;Hilary</description>

      <description>I cannot use mediainit because the disk is a root disk and there is no mirroring.  However what i have is a ignite tape which i created yesterday, thus what i will do is try and have the disk replaced and then boot from ignite.  In addition at the moment the disk is claimed and i have all the logical volumes available.&lt;BR /&gt;&lt;BR /&gt;Thanks guys!  i have had great response....!  :)</description>
