Can anyone provide or point me to a document that gives instructions on how to
recover a system using an Ignite UX make_recovery tape ?

Also, am I correct in assuming that all I would need is this and a full system
backup to restore my system ?

So long as your are restoring to the original hardware configuration, this is
correct. There is a man page on make_recovery that should answer all of you
questions. Personally, I always use the preview mode and then manually alter
the list file and resume the backup. I also use teh check_recovery option to
help keep my copies up to date. finally, make sure you have the latest PAX
patch. Without it, the make_recovery will seem to work (no error messages) but
you will not be able to boot from the recovery tape.
