    <description>Wes,&lt;BR /&gt;&lt;BR /&gt;This problem can also be caused by corruption of the IPD (Installed Products Database).  Have you tried to reduce the size of /var lately, maybe by manually removing any files under /var/adm/sw?&lt;BR /&gt;&lt;BR /&gt;Try running the check_patches utility (hopefully you have it) and see what it reports.  Maybe you could post the report or the output from the swlist command.&lt;BR /&gt;&lt;BR /&gt;Best Regards,&lt;BR /&gt;&lt;BR /&gt;Dave</description>

      <description>On a HP K580 running HP-UX 11.11, failed to add secuity patches. Swinstall gave this error message during the Analyzation Process:&lt;BR /&gt;&lt;BR /&gt;Internal error. The controller phase is out of synchronization.&lt;BR /&gt;Current phase is 2 and the new phase is 2. This error ocurred on &lt;BR /&gt;function "SMC_ChangePhase, source file "smc_change.c", line&lt;BR /&gt;1557. Please give this information to your Hewlett-Packard support contact..&lt;BR /&gt;&lt;BR /&gt;You should exit the user interface immediately. Proceed at your own &lt;BR /&gt;risk!&lt;BR /&gt;&lt;BR /&gt;Do I have a Hardware issue?&lt;BR /&gt;&lt;BR /&gt;Thanks,&lt;BR /&gt;&lt;BR /&gt;Wes&lt;BR /&gt;&lt;BR /&gt;&lt;BR /&gt;</description>

      <description>I'd say you have a hardware issue and should contact HP.&lt;BR /&gt;&lt;BR /&gt;Its pretty rare that they actually give you a message that says you must call support.&lt;BR /&gt;&lt;BR /&gt;There may be a patch, but HP may need some data to sell you precisely what patch you need.&lt;BR /&gt;&lt;BR /&gt;SEP</description>
