Why it is "UNKNOWN" instead of lpfc in Path field and is "unknown" in the I/O Paths field in the powermt display?

whhat is the output of this commands :

powermt check

powermt config

powermt display dev=all

powermt save

PS : sometimes its very good to reboot the server.

I strongly suggest you upgrade to PowerPath 5 (latest version is 5.3.1 SP1), 4.5.1 is no longer supported.
Also make sure the HBA drivers are listed in EMC Storage Interoperabilty Matrix, if you don't have access to EMC Powerlink to check, please let me know:
1) HBA Model
2) OS Version
3) HBA Driver Version
4) Storage Array Model and Firmware (Code) Version.
