HSZ70 - Newly created MirrorSet goes into FailedSet

Have a RA7000 with single HSZ70 controller currently presenting 4 mirrorsets to Tru64 V4.0F. Everytime I create another Mirrorset using any combination of the available disks it puts the mirrorset into a failedset, although the new mirrorset is presented and accessable with Tru64.
e.g > add mirror m5 disk10000 disk20100
>show failedset - this then lists m5

Any ideas ??
