Latest HP D8602B driver is afcw2k.sys version 3.0.4107.0 for Windows 2000, 8/8/2001 but it is not available on public HP site, only on certified cluster installer site (password protected). If you do not have access to this site you can try last version of Agilent's driver. Go to www.agilent.com select products, semiconductor products, HBAs. HP D8602B is from agilent.
Another possible cause of problem may be RS12 box. You can try moving Array controller in second slot and use port 1 on controller.
Most common cause for such a problem is incorectly defined fcarray user account. fcarray user account must be member of local addministrators group. If you have made it member of domain admins group (which make him member of local administrators group) check if domain data are replicated between all domain controllers. If your connection is through FC hub you can try direct connection between HBA and port 0 on FC Array to eliminate possible hub problems.
