I had the same problem with brand new RS12. The cause of error was faulty SCSI card in RS12. If you have spare one change it, if not call Support Provider in your area.
HI all,
I have a Rs12 (D5989C) connected on lc2000 with a netraid 1m (fw h108). there are 2disks P1166A configured in raid 1.
When I run (in netraid express tools) the option initialize it stopped at 66%. In case of initizialize successfull, under disk administrator of Nt4_0 server (sp6a), the format doesn't work. It makes several time (1 night) to give me a message about the impossibility to format the partition.
The format in netraid express tool, doesn't work.
I try to connect that disk in netserver, and all is ok.
I changed internal and external SCSI cable of RS12.Same problem.
Can you help me?

Call HP to have them check your disks. They could be bad.
