      <description>Since you are having too many path failures at once, I am suspecting either a fiberoptic cable or a FC  HBA on either end is going bad, maybe intermittently. It is hard to tell.</description>

      <description>Why the host ports? surely if you have other Hosts running ok this is not the case. Log in to your switch and see if there are connection issues on the port going to the Host with the connection issue and then check the port going to the EVA host port, this will tell you which side of the SAN switch the issue lies. You say you swapped the FC cable and the HBA, what about the SFPs they are more likely to be an issue than the EVA host port.</description>
