12/24 Dat - Unable to find media

Have been using the 12/24 DAT tape backup (with DDS-3 Maxell and Sony tapes) and BrightStor(Arcserve) software reliably for several years now. Currently and unexplainably I am receiving 'Media Errors' when trying to format new tapes also, 'Unable to find any Media' when running scheduled backup jobs.
I run cleaning tape weekly and have ran 4-5 times and still receiving 'media errors'. Any thoughts as to the problem would be appreciated or confirmation that the tape head has seen better days.

John,
install and run HP LTT http://h18006.www1.hp.com/products/storageworks/ltt/index.html and generate support ticket for the drive. Look into this support ticket thoroughly. If you'll see that it recommends to recalibrate heads then do so running DDS3 drive recalibration script (you may need to download it first from FTP site using LTT's 'Get files from the web..' menu)

I've run the HP LTT but am unable to view the support ticket. Error:"Diagnostic function encountered an error...".
Any thoughts?
