Robotic Library Errors

I have a DAT40X6 (the newer one with the H910 firmware).
Connected using an adaptec SCSI cable to an Adapted 29160LP SCSI controller.

More than 50% of the time when we run a backup Backup Exec shows a "Robotic Library Error" and sets the autoloader to offline. I eject the magazine and one tape (it varies which one) is still in the drive. I reinsert the magazine and eject it and the missing tape comes with it. I have switched SCSI controllers and cables and no difference.

Any ideas?
