    <description>Accoring to my example post the latest SA FW is version 2.84. I would consider to upgrade first. (FW is on the IPF offline diags CD available on software.hp.com)</description>

      <title>CISS Smart array controller error.</title>

      <description>on rx4640 machine I am getting below error while booting.&lt;BR /&gt;&lt;BR /&gt;========================================&lt;BR /&gt;CISS: Raid SA controller on hardware part 0/1/1/0/4/0 has detected fetal IO.&lt;BR /&gt;&lt;BR /&gt;logical block add -0&lt;BR /&gt;Logical block count 16&lt;BR /&gt;Logical command 0x20&lt;BR /&gt;Fetal Drive Bus 1&lt;BR /&gt;Fetal Drive ID 0&lt;BR /&gt;==========================================&lt;BR /&gt;I have two hard disk when I boot with "boot -lm" ( LVM maintenance) I am able to go to single user mode and able to activate all the VG and mount LV's. After mounting all partition I tried to execute below commands but I got same error and not getting prompt back.&lt;BR /&gt;&lt;BR /&gt;===========================&lt;BR /&gt;sautil /dev/ciss2&lt;BR /&gt;saconfig /dev/ciss2 -F off&lt;BR /&gt;================================&lt;BR /&gt;&lt;BR /&gt;I am having data on this disk so I don't want to take risk by clearing SA configurations please help. How to resolve this?</description>

      <description>Looks like you have a problem with a drive.&lt;BR /&gt;&lt;BR /&gt;Can you access ORCA (press ESC followed by 8 during boot) and check the status from there?</description>

      <description>Hi I am able to access ORCA (Option Rom Configuration for Arrays). And I can select all the options and I have seen "view logical Drive" it is showing good.</description>

      <description>And also I am able to boot with option "boot -lm" and there I activated vg00, vg01 and mounted all logical partition its fine I am able to access. But normal boot is not happening.</description>

      <description>IMHO 1 out of the 2 drives is blocking everything. Most likely the lower disk.&lt;BR /&gt;&lt;BR /&gt;I would try to power off the server, remove this disk and power on again. Ignore the message regarding a missing drive, the server should start. If not successful, power off again and try the other drive.</description>

      <description>I tried to boot removing one disk after another, but I am getting error "boot disk failure" .  I followed below steps.&lt;BR /&gt;&lt;BR /&gt; a.) I booted system in LVM maintenance (boot -lm) mode and ran command "setboot -p /dev/disk/disk3 -b on" -- Disk which is having SCSI ID 0(lower disk )&lt;BR /&gt;&lt;BR /&gt;  b.) No alternate boot path set&lt;BR /&gt;  c.) Edited /etc/lvmrc to activate only /dev/vg00&lt;BR /&gt;  d.) rebooted the system by removing one hard disk i.e SCSI ID 1.  But I am getting error "boot disk failure"..</description>

      <description>Yes both disks are on "Smart Array controller" only. &lt;BR /&gt;&lt;BR /&gt;  But my doubt is if it is a problem with Array controller why it is booting in "boot -lm" . And I am able to mount all the disks there.  If I am booting system normally this error is coming.. !!</description>

      <description>3.17 is the firmware version.&lt;BR /&gt;&lt;BR /&gt;And Smart Array controller is 6400-128 (version 2.34)</description>

      <description>ROM configuration for array utility is running fine. And status of the 2 Logical Disk its showing OK.</description>

      <description>I resolved this error by Removing Logical disk from ORCA and added it again and re-installed the OS.  Since node was in cluster I have not faced much difficulty in retrieving data. But if you have any Data which is important , it might become very difficult to bring back the machine. &lt;BR /&gt;&lt;BR /&gt;Thanks for support !!!</description>
