OA version looks Old.
Try updating the OA to the New Firmware.
Version 2.20 and can you check does it fixes it.
http://h20000.www2.hp.com/bizsupport/TechSupport/SoftwareDescription.jsp?lang=en&cc=us&prodTypeId=329290&prodSeriesId=3188465&prodNameId=3664326&swEnvOID=1005&swLang=8&mode=2&taskId=135&swItem=MTX-f450191ce2b141d7834a9b5c55

Probably this problem is known. Time to time some BL460c loses power control. No power on via iLo no even button press on the front panel do not turn server on. The only know solution is the reinserting blade in to enclosure. But this solution is not remote enough ;-)
The only command that shows some diagnostics was:
hpiLO-> start /system1
status=2
status_tag=COMMAND PROCESSING FAILED
error_tag=COMMAND ERROR-UNSPECIFIED
Failed to power on server.
All other actions to power on server are silently do nothing.

Hi,

What is the version of ILO and OA you are using?

From the OA CLI you can execute command reset server Bay number which will serve your purpose.

Check the entries in the IML of the blade and also the event log in OA to see what's happening...
