How to add custom index?

Hello, I am trying to add a custom index to monitor our FW temperature. I added the necessary MIB to IMC and can select the OID when attempting to add it to the "Instance Label" and the "Formula" but I do not know what to enter for the "Type". I have tried the following:

It seems to pass the "Test", but then if I enter the IP addess in the Collection Instance and try to the resolve the select instance, an error "Failure, Unsupported feature or lack of necessary configuration" appears.

Following image shows the result from the SNMP GET in the MIB Management tool.

Any help would be greatly appreciated.
