HP 3PAR S400 - Failed PD, Two PD's referencing same Cage, Mag, Drive

I got two PD's 91, 141 looking at same location 2:2:3, had a disk fail PD91. Replaced but resume failed saying the new PD141 failed.  Tried unmark and rerunning servicemag start but command will fail and not succeed.  
 
If you put the -PDID # is says to many disks on SLED

What 3PAR OS version are you on?  2.3.1 or 3.1.1?

Have you talked to Support yet, your system seems very sick with PD 91.
