You have the 1/8 Autoloader, correct?
Error 89 is a timeout error. The first thing I would check is the SCSI cables. If there appears to be no issue there, try running the diagnostic system test, and see if error 89 comes up there. If so, the unit may have a drive going bad.

Also, try the ff. for isolation:
- Update your tape driver using Veritas Backupexec device driver installer (see below) Just make sure your using BENT90 rev. 4454 and not 4367.

As has been mentioned before, you have a SCSI issue. Check your SCSI for any missing or bent pins. Recede it. If you've a spare, swap it.
