C3000 KVM module/ OA loses connection to individual blade servers

We are experiencing issues around the local connection by a Pullout LCD monitor that is connected to an external KVM switch. The KVM switch connects to the C3000 KVM module by mean of a CAT5E cable and a USB module.... the connections work on some of the blade(s) but not on others; when trying to connect to other blades, using OSCAR or Analogue Menu, you try and connect to the blade and it kicks you back out to the green blade menu selection screen

Anyone with ideas on why this is happening would be great
