HP B-series 8/12c SAN and QMH2562 8GB Mezz Card
I have a QMH2562 8GB Mezz Card card in a BL460cG6 in conjunction with a HP B-series 8/12c SAN Switch. The issue is that SAN switch only sees one port on the QMH2562 card, which on the ESX server is showing 2 ports, so does the port mapping of the "B-series 8/12c SAN Switch" interconnect, but the 2nd port on bay 4 does not have an "OK" on it like the 1st port does on bay 3 - under the interconnect port mapping screen. Is there a limitation of HP B-series 8/12c SAN Switch to only access one port?

after some research I think I know the issue. I am only using 1 "B-series 8/12c SAN Switch" in my enclosure. I think that I need to have a 2nd "B-series 8/12c SAN Switch" in my enclosure for the 2nd port to show up...

by putting in the 2nd SAN switch, I was able to utilize the 2nd port of the QMH2562 card.
