QLogic QMH2462 4GB FC HBA 'disappearing act' in BL460c

Installed dual-port HP QLogic QMH2462 4Gb FC HBA in 2 of my blade servers (Bl460c)for connection to our HP EVA 6000. Problem: One of the blades has a strange issue with its QLogic HBA doing a 'disappearing act' intermittently during boot. Planning to deploy RHEL AS 4 on both but not loaded anything yet at this stage. So during boot, right after the smart controller BIOS, I sometimes get the or option, and sometimes I dont. Please, would appreciate any pointers!

Well ...You must have tried allready to reseat the card ?

Yes I did, first thing!
Also confirmed same issue on my second blade server today. I have a feeling it might be forware related but I've no idea which firware I should start with (Blade or HBA).

Thank you. I had a look at both before but couldn't see how I could get an updated firmware so ignored it. However, now that you've highlighted the links again, I will revisit & advice - espeacially sansurfer; I'm hoping it will provide me necessary updates I might reqiure to get my HBAs to settle down.

Call HP support for replacement of the HBA.
This is a known issue.
