Upgradation of form30 to forms4.5


hi every body,

What are the step involved in upgrading
forms30 to forms4.5.

What are the script to be run for creating a base table to store the forms in database.

I have tried to upgrade some form with insert=Y option (ie inp files) but it is giving error:
Table or view doesn't exist.
