Device file not created on boot

Hello,
I have an HP-UX 11.0 system that does not create device files for an HDS 9960 on boot.
I have to run insf -e manually when the system is up and running in order for the devices to be created.
I have a feeling that it has something to do with the ioconfig file but I ran ioinit -i to check the consistency of the file and the kernel structures and everything is clean.
Any suggestions?
