the server detected lost path to Lun MPIODisk11, MPIO quickly detect this and diverting IO access to surviving path to MPIODisk11.
THis is good example of redundancies set in your storage implementation.
I believe you would probably have 2 fibre cards connect to 2 diff san switches and separate connection to storage controllers.
To futher investigate, you can check at your end of the following:
1) from EVA command view, check controller status, if any of the controller fail, proceed with replace the defect controller.
2) check san switches and ensures no gbic failed or san switch hang.
3) check for physical cables, ensure they r still attached.
4) check you fibre card status from your server, identify if its faulty or failing card.

No controller failure events on EVA, no ports events on the SAN Switch end.

And also check at server side, check fibre card status. Ensure firmware is similar for both cards. What is current firmware for both cards now?
