From where we can find that the Disk is failed in EVA, and for replacement which things are necessary. RAID level 5 is used.

You can see this from Command View EVA.
Go in CV EVA go to HSV Storage Network and then go to Hardware and then go to disk enclosure and see the operational state.
You can see the individual status of the disk too from CV EVA.

If the disk is failed replace with another one immediately. You can do this online.

I recommand you call HP-Engineer to replace the drive. You can idientify the prob and log a call with hp,because I dont think Hp will allow others to replace the spares of EVA. I guess your storage is under Warranty support with HP.
