I'd be interested in seeing what a pvdisplay of the PV looks like, as well as a vgdisplay of the vg.
You may simply have corrupt PVRAs (lvm headers) that can be repaired with a vgcfg restore, one the /etc/lvmconf/ files for vg01 still exist correctly.
strings /etc/lvmtab also just for verification..
You did do a pvcreate right?

It's the IDE to SCSI converter adapter that is probably the issue here.

I don't think that vgcreate command takes two disks at a time during creation of the VG. First you have to create the VG with the first disk then you have to do 'vgextend for the rest of the disks. vgextend can take the multiple disk as the argument but I don't think that vgcreate takes multiple disks as the argument.
