QL2300 error 129

Hi,
we have similar problem like this:

Our server config:
- BL460 G6, QMH2462 (latest HP driver and FW), W2K8 x64 SP2, SQL2008
- no HW errors in System Mgmt Homepage
Our storage config:
- EVA4400 (latest XCS), FC blade switch 4/24
- no HW errors in Command View

After installing SP2 on W2K8, the errors appears in Win event log and 2 of 4 presented vdisk from EVA sometimes disappeares. After server restart vdisks are back.
We changed QMH2462, again update its driver and FW, but problem remains.

Note: We have other 3 servers (1 x Win, 2 x ESX) with other vdisks from EVA and there is no problem.
