Syslog Error : vmunix: SCSI: Unexpected Disconnect -- lbolt: 313573669, dev: cb083002, io_id: 800006

I am working on a N Class HP 9000 Server with an FC60 disk array.
We keep getting these errors in syslog. Also in the amdsp, one of the disk 5:4 is not recognized, although there are no errors in amdsp, no rebuilding, no hot spare being used.
The sam gui shows a blank space in the FC60 trays display.

Have you executed

/opt/r
esmon/bin/resdata -R 185401351 -r /storage/events/disk_arrays/FC60/AM60Srvr -n 185401355 -a

The output of this command will point you to a website, which gives more info

Yes, but the website still says same error.

We've done that several times in the past few days. ArmSrvr still keeps stopping, gets restarted on its own at times, or we have to manually start it, and these errors keep accumulating in syslog.
