Syslog Error : vmunix: SCSI: Unexpected Disconnect -- lbolt: 313573669, dev: cb083002, io_id: 800006

I am working on a N Class HP 9000 Server with an FC60 disk array.
We keep getting these errors in syslog. Also in the amdsp, one of the disk 5:4 is not recognized, although there are no errors in amdsp, no rebuilding, no hot spare being used.
The sam gui shows a blank space in the FC60 trays display.

The resource monitor will continue to attempt to connect to the AM60Srvr at regular polling intervals. When the AM60Srvr is restored, the connection will succeed and normal operation will continue. Until the AM60Srvr is operating normally, it is not possible to obtain the current status of any of the disk arrays.

We've done that several times in the past few days. ArmSrvr still keeps stopping, gets restarted on its own at times, or we have to manually start it, and these errors keep accumulating in syslog.
