...one or more drives has only a single path of access to it.
Questions: should I exchange the drive? Why armlog shows it as good/ready? How failure of a drive leads to losing of second path? Why SDM does not show warning state of the VA?

I have faced same problem.
Below are the steps to resolve this issue.
1) Upgrade Command View SDM to 1.07
2) Upgrade VA Controller to A120 at present it is A000 which is very old that also can give u problem.
3) Upgrade Disk Firmware to HP09 at present it is HP08.
If this doesnt solves ur problem then post ur armdiag output or logprn output to study more about this VA.

Your C1 & C2 Host Port2 is of 1Gb where as Host Port1 is 2Gb why is this so??
& ur C1/H2 & C2/H2 Link is Down
can u check FC cables weather the link is proper or not??
This could create a problem.
& upgrade DS2405 Firmware to HP04.

Port 2 are not connected to HBAs so their link is down.
The question remains: should I exchange the drive that logged media error and now has amber LED on?

SolutionThe drive probably disconnected and reconnected its self. Have you heard of the famous VA7400 has connection problems issue. Well you probably have it.
You can either have the drive replaced, but i doubt HP will do this. Remove the drive. Check if the VA starts a rebuild. If so do not plug in the drive till it is finished the rebuild. Plug in the drive after rebuild.
Watch the status and make sure rebuild completes again.
