My SAN environment is├п┬╝
RA8000/HSG 80├п┬╝ connecting to a San Switch 8.
AS4100/ Tru64 5.1B/KGPSA-CA, connecting to the San Switch 8
Switch led: steady green for 2 HSG connection ports,
Yellow for the port connecting KGPSA-CA when AS4100 in normal console >>> but it turns green when AS4100 is in diag mode or when tru64 booted.
The detail status info is in the attachment.

As Stephen suggests, what do you see on the SAN switch ?
If you telnet on to it, can you see the WWNs of the HSG80s and the KGPSAs when you issue the switchShow command ?
If you can, check whether any zoning is configured, which is limiting the access of the 4100 to the HSG80s. cfgShow

Is there any way to reset the switch to its factory default?
By the way, what're detail orders to have HSG80 creating connections automatically? I mean steps about turning on device/server power, plugin into switch, and etc.

Have you tried logging in with admin/password and root/password
These were the defaults. If they have been changed, then it can be 'interesting' to get them reset...

The initial webpage displayed has a line somewhere near the bottom which tells you name of the active configuration, IIRC.
If you don't know the IP address you'll have to hook up the serial console, and look at the output whilst the switch is booting.

The HSG needs to receive a 'Fabric event' to do that automatically. Usually, a server reboot and/or a scan via WWIDMGR is sufficient.
However, from the output of

it is apparent that the AlphaServer does not 'see' the HSG ports. It could be a switch zoning issue as has already been suggested.
It could also be a lack of LUN 0. The HSG is running in SCSI-2 mode with the CCL being disabled and it is not obvious if you have a Unit 'D0'.
What version of Tru64 Unix is running on the Alpha? Or is there another reason you don't use SCSI-3?

You might also want to check the firmware revisions on the HSGs and the KGPSA.
The version of ACS on your HSG80s (8.5-0) is very old. That *might* be causing you problems...

I modify the switch's IP and use web to check it. But I can not check its zoning info as both default admin and root passwords have been changed (not password).
Is there any way to bypass login, or do factory reset, or reload its firmware without any password?
