Machine becomes Unresponsive -> Array or Disk Error

Recently I had a Smart Array 5300 Controller go out in my ML350. I replaced the Controller and all went well for about 2 weeks.

Now every couple of days my ML350 will become unreponsive to the network and to the terminal screen. I have to reboot the machine. When the machine comes up, after initializing the arrays (Drive C is on a RAID-1 and Drive D and E are on a RAID-5), it displays a message about finding a new array and I have to push 'F1' to continue the boot process.

After logging into Windows 2000 Adv. Serv., I check the 'Event Notification' log under the properties of the Controller and it gives me this:

Next I run 'Drive Array Diagnosis' and the log it spits out gives me this:

SLOT 2 SMART ARRAY 5300 CONTROLLER ERROR REPORT:

Array accelerator parity read errors: 100

There is clearly a problem. Is it my array contoller or disk drive? FYI, I have updated to the latest firmware and drivers.
