Anyone know if the 501575-001 is the same as a 381513-B21? Are both of these P800 with 512MB BBC? Only the former is listed in the PartSurfer, but only the latter is listed in the quickspecs for a DL385 G5.

Don't know the answer to your specific question for certain but I believe the -B21 is used for parts ordered through the normal system and -001 is used for repair part numbers.

Solution
The answer seems to be YES, both numbers go to P800 with 512 MB cache.
I also have to mention that *-00? is the spare part number and it can be a re-repaired one,
*-B21 is a sales part number so it should be a new one if not mentioned by the seller to be a re-marketed one.javascript:postAnswerSubmit('submit');

NO, they are NOT the same part. The 501575-001 is only the board. The 381513-B21 includes both the cache and the batteries.
